My observation is that many models straight up dislike facials and once they get splattered they clearly lose their mood and just want to get over with it. This may be one reason to include them only at the end.
Not having facials these days is a byproduct of this streak of releasing the first parts of the shoots. Once they switch to the second parts (after or before Kechteny, lol) all updates gonna have facials for sure.
As for variety, I think they are very careful to make only minimal changes at a time and see the results. Looks like the current template works the best and any major deviation causes uproar from certain groups.

All of this but especially “not deviating from the formula”. They’re not gonna please everyone, and the current strategy seems to be working. I do think they will try to switch up things like the random blue box gloryhole, or megabukkakes, but the ~50+ loads first scene ~50+ loads incl facials in the second scene is working for them.
